Onychomycosis nail fungus

Several commercial products are available to treat onychomycosis, including griseofulvin, itraconazole, Terbinafine, as well as ciclopirox 8% nail lacquer (Penlac). It takes 10 to 18 months for griseofulvin to work and has poor cure rates. Podiatry Oral therapy with itraconazole and Terbinafine also present problems with drug interactions and liver toxicity.  

A patient with compromised liver function and a GERD patient on a PPI will not qualify for oral itraconazole.  

For patients who cannot use antifungals, Penlac is the only commercial topical treatment available.  However, Penlac reportedly has only a 14% cure rate after several months of use.  

Having a compounded prescription medication from a compounding Pharmacy may overcome these problems and avoid the need for extensive liver monitoring.  

Antifungals can be dissolved in dimethylsulfoxide (DMSO) which has superior nail penetration and can be delivered directly to the infection site.  Terbinafine and azole antifungals are examples of antifungals that can be compounded in DMSO.

Possible formulas -

  1. Itraconazole 1%/Ibuprofen 2% in DMSO nail polish 
  2. Itraconazole 1%/Undecylenic Acid 17% in Tea Tree Oil-DMSO nail polish 
  3. Itraconazole 1%/Undecylenic Acid 17%/Salicylic Acid 10% in Tea Tree Oil-DMSO nail polish 
  4. Ketoconazole/Tea Tree Oil/DMSO  Antifungal Solution, and
  5. Terbinafine 1.67% Topical Solution


The classic treatment for warts is salicylic acid, used topically.  Our experienced compounding podiatrist in Newport, can prepare some unique treatments for warts involving Cimetidine, trichloroacetic acid, and 5-fluorouracil.  A dose of 40 mg/kg orally of cimetidine has been used to treat multiple warts that are recalcitrant.

Possible formulas -

  1. Cimetidine 5%/DDG 0.2%/Tea Tree Oil 10%/Ibuprofen 2% transdermal
  2. Salicylic Acid 40% Ointment
  3. Salicylic Acid Compound Collodion, and
  4. Salicylic Acid 28% Polymeric Substrate, and
  5. Trichloroacetic Acid 2%/Salicylic Acid 60%

Non-Surgical Nail Removal

Urea 40% ointment Diabetic Neuropathy

Numerous medications are commercially available to treat diabetic neuropathy and help return sensitization.  Some drugs used to treat diabetic neuropathy are amitriptyline, baclofen, ketamine, gabapentin, and clonidine.  

These medications, which are not commercially available, can be combined topically to benefit diabetic patients. Combinations of topical medications have the advantage of fewer adverse effects as well as using one medication rather than several.

Possible formula -

  1. Baclofen 5%/Ketoprofen 10%/Lidocaine 5%/Gabapentin 5% transdermal 
  2. Ketamine 10%/Gabapentin 6%/Clonidine 0.2%/Lidocaine 2% transdermal 
  3. Ketamine 10%/Gabapentin 6%/Clonidine 0.2%/Nifedipine 2% transdermal 
  4. Gabapentin 6%/Clonidine 0.2% transdermal
  5. Clonidine 0.2%/Gabapentin 6%/Ketamine 10% transdermal

Hyperhidrosis of the Feet and Foot Odor

Aluminum, atropine, scopolamine, formaldehyde, methenamine, and glycopyrrolate form the basis of many products prepared to treat hyperhidrosis feet.

Methenamine is a pro-drug that is converted to the active drug formaldehyde.  Many over-the-counter antiperspirants contain aluminum chlorohydrate

Possible formulas -

  2. Methenamine 5% Deodorant Stick
  3. Methenamine 5%/Benzalkonium Chloride 2%
  4. Aluminum Chlorohydrate 5% and 10% Topical Antiperspirant Cream

  7. Formaldehyde Roll-on Deodorant
  8. Glycopyrrolate 0.25%/Tea Tree Oil 2.5% Deodorant/Antiperspirant

Inflammation and Pain

Most topicals for inflammation and pain due to sprains and strains can be made from various NSAIDs in PLO containing ketoprofen.  NSAIDs have the most clinical support of all the medications used topically in podiatry.  We can combine the NSAIDs with a muscle relaxer and topical anesthetic at our Newport Podiatry center if required.

Possible formulas -

  1. Ketoprofen 10%/Cyclobenzaprine 1-2% transdermal
  2. Ketoprofen 10%/Lidocaine 5%/Bupivacaine 2% transdermal

Wound Healing and Circulation Improvement

Nifedipine is used in concentrations of 0.2% to 10% PLO gel in an effort to enhance circulation in areas of ischemia. Our podiatrist Newport beach, uses higher concentrations, but the lower doses are a better starting point due to the hypotensive properties of this drug.  The vaso-dialation properties that make it work well orally in diseases such as hypertension, angina pectoris, and Raynaud’s syndrome could also work well as an agent to rub on ischemic areas and diabetic ulcers to increase blood flow. Despite its light sensitivity, transdermal nifedipine PLO should be dispensed in an amber bag. During the preparation, patients’ blood pressure should decrease, and wounds should be monitored for signs of healing (redness, vascularization, and dryness around the wound).
